General Licensing Application Packet

Nevada Business Registration Form
The
Nevada Business Registration Form (NBR) is the main part of the application and must be completed. An advantage of this form is that it is also utilized by most state agencies; therefore you do not have to repeat all the pertinent information to each agency. The NBR has specific directions, please be sure to read it carefully and completely fill out the document.

The
Business License Supplement is a form for the Business License Department and must also be completed.

Worker’s Compensation
Industrial Insurance requires an Affirmation of Compliance with Mandatory Industrial Insurance Requirements, which indicates you have complied with industrial insurance coverage requirements in Nevada (NRS616). The form needs to be completed; "signed in the presence of a business license technician”; and submitted to the Business License Office prior to the issuance of a business license. If this form is mailed or faxed to our office, it must have the signature notarized. GENERAL Business Licenses, Permits and Registrations
Application for Reflexology/Drugless Practitioner Business License Form
The Application for Reflexology must be completed and notarized before a license can be issued.
Charitable Registration
The Charitable Registration packet includes application forms and instructions to register as a charitable organization.
Charitable Registration Solicitation Form
The Charitable Registration Solicitation Form is required if your organization is holding events to solicit charitable funds.
CONTRACTORS - Multi-Jurisdictional Supplemental for New License Application Form
Completion of the Multi-Jurisdictional Supplemental for New License Application Form is required of all contractors. This form allows an applicant for a Contractor license to select other participating jurisdictions (Henderson, Las Vegas and/or North Las Vegas) in which the applicant also intends to conduct business.
CONTRACTORS - Multi-Jurisdictional Supplemental and Change Request Application Form
Completion of the Multi-Jurisdictional Supplemental and Change Request Application Form is required of contractors that (1) intend to conduct business in one or more participating jurisdictions (Henderson, Las Vegas or North Las Vegas) and wish to add the jurisdiction(s) to the County business license, or (2) will no longer be conducting business in one or more participating jurisdictions and wish to remove jurisdictions from the County business license.
CONTRACTORS - Multi-Jurisdictional License - Notification and Acknowledgement of Transfer of Primary Jurisdiction Form
The Notification and Acknowledgement of Transfer of Primary Jurisdiction form must be used by contractors when their business location changes from one participating jurisdiction (Clark County, Henderson, Las Vegas or North Las Vegas) to another. This form will be used by the participating jurisdictions to transfer control of your license to the new Primary Jurisdiction.
Complaint Form
All complaints must be received in writing. The complaint form contains all the information we need to investigate a complaint including a return address and a fax number.
Fictitious Firm Name Form
Need to file a Fictitious Firm Name? Read about the County Clerk requirements if you plan to officially "name" your business. There is some excellent information for you on their website and you will have all the information you need to file your fictitious firm name. This form needs to be filed prior to applying for a Business License.
Request for Public Records
Request for Public Records can be obtained by completing the Public Records Request Form.
Request for Duplicate License Form
Obtaining a duplicate license can be accomplished by completing the Request for Duplicate License Form. License reprint fee is $5.00
Request to Surrender General Business License Form
Surrender of your general business license can be accomplished by completing the Request to Surrender Business License Form.
Vendor Registration
If you are doing business with Clark County, University Medical Center (UMC), Department of Aviation (DOA), or Clark County Water Reclamation District (CCWRD) bid, RFP, or RFQ, you will be required to complete the Vendor Registration form.
