If applying for a Dust Control Permit in the jurisdiction of unincorporated Clark County, read this notice.
Dust Control Permit Application - Forms DCP 01, DCP 02, DCP 03, DCP 04, DCP 05, A-1, and A-2.
Supplement to the Dust Mitigation Plan - B-1 and B-2 - These two pages contain the instructions that you will need to complete the supplement.
Dust Control Permit Renewal Form - DCP 10 - This form is to renew an expiring Dust Control Permit.
Permit Modification Form - DCP 06 - This form is required to change information on your Dust Control Permit.
Project Completion Form - DCP 08 - This form is required to close your Dust Control Permit.
Permit Address/Contact Change Form - DCP 06A - This form is used to change the Permittee's address and/or other contact information.
Transfer of a Dust Control Permit - DCP 11 -This form is required to transfer the permit or change the property owner.
Dust Control Sign - Information and requirements for the required signage.
Dust Palliative Information - DCP 07 - This form is required if you plan to use a Dust Palliative for long-term soil stabilization.
Dust Control Monitor - DCP 09 - This form is required for projects with over 50 acres of disturbed soil.
Daily Record Form - A-3 - This form contains the information you must maintain daily.
Soils Analysis for Projects 50 Acres or More - DCP 12 - This form is required for all permit applications of 50 acres or more.
