Application/Permit for Alcoholic Beverage Tasting
The Application/Permit for Alcoholic Beverage Tasting form is only applicable to grocery stores licensed for the sale of package liquor and/or package beer wine and spirit based products. Click here to visit our Special Events Division for more information. 
City Jurisdiction Gaming Application
This one (1) page Application for Gaming License that is required for all applicants whose business location is within an incorporated city in Clark County. If you do not know if your business location is in the jurisdiction of the Clark County Department of Business License, you may refer to the Jurisdiction Locator.
Liquor Location Suitability The Liquor and Gaming Licensing Board provides, through Clark County Code 8.20.115, a means to approve a liquor location (not a business or a person) suitable for a specific type of liquor license prior to that location having an existing business located thereon. The Board can grant a preliminary determination of location suitability to an applicant, who then has a specific period of time in which to build their facility, undergo personal suitability to hold the requested privileged licenses, obtain the necessary building, fire, and health department inspection approvals, obtain the Licensing Boards approval for the actual liquor licenses, and open for business.
Liquor and Gaming Location Suitability The Liquor and Gaming Licensing Board provides, through Clark County Codes 8.04.075 and 8.20.115, a means to approve a liquor and gaming location (not a business or a person) suitable for a specific type of gaming license prior to that location having an existing business located thereon. The Board can grant a preliminary determination of location suitability to an applicant, who then has a specific period of time in which to build their facility, undergo personal suitability to hold the requested privileged licenses, obtain the necessary building, fire, and health department inspection approvals, obtain the Licensing Boards approval for the actual liquor licenses, and open for business.
Liquor Caterer Event Permit All liquor caterers licensed by the Clark County Department of Business License must submit a completed Liquor Caterer Event Permit form and associated fees at least two (2) working days prior to the scheduled event for which they will provide alcoholic beverage service. City liquor caterers (North Las Vegas, Henderson, and Las Vegas) must show proof of valid city licensing and submit a completed Liquor Caterer Event Permit form and associated fees to conduct an event in the unincorporated areas of Clark County. Incidental Activity Permit Application The Incidental Activity Permit Application is used by liquor establishments licensed for the on-premises consumption of alcoholic liquor and cannabis consumption lounges to allow specific types of live entertainment at the establishment.  Pursuant to Clark County Code 8.20.020.147, a liquor establishment must first obtain a permit prior to offering any live entertainment, which includes, but is not limited to, the performance of acts, music, speech, dance, acrobatics, disc jockeys, karaoke, or display. 
Pursuant to Clark County Code 8.65.164(c), a cannabis consumption lounge offering Live Entertainment must first obtain a permit prior to offering any live entertainment, which includes, but is not limited to, the performance of acts, music, speech, dance, acrobatics, disc jockeys, karaoke, or display. 
LVMPD Investigation Referral for Adult Entertainment Cabaret All applicants for adult entertainment cabaret business licenses in the unincorporated areas of Clark County are required to complete an LVMPD Investigation Referral for Adult Entertainment Cabaret, which is forwarded to the Las Vegas Metropolitan Police Department for investigation.
Request to Surrender Gaming License Form Surrender of your gaming business license can be accomplished by completing the Request to Surrender Gaming License Form.
Request to Surrender Liquor License Form Surrender of your liquor business license can be accomplished by completing the Request to Surrender Liquor License Form.
Request for Temporary Liquor License The proposed new owner of an established, licensed, and operating business with a liquor license may request the Department of Business License to issue a Temporary Liquor License to protect the goodwill of the business with their existing clientele. Any request for a temporary liquor license must be accompanied by the former owners consent.
Off-Premise Liquor Permit Application All businesses (except liquor caterers) licensed for liquor by the Clark County Department of Business License must submit a completed Off-Premise Liquor Permit Application form and associated fees at least two (2) working days prior to the scheduled event for which they will provide alcoholic beverage service. Charitable/Nonprofit Organizations must show proof of valid charitable solicitation registration with the Department and submit a completed Special Event Permit form to conduct an event in the unincorporated areas of Clark County. Transfer of Interest The Transfer of Interest form allows an existing licensed business to notify the Director of Business License of any proposed transfer in the ownership of the business. It is required to be signed and notarized by all parties involved in the transaction, and supporting legal documentation as to the sale/transfer must accompany the form.

Authorization to Release Information
Each applicant for a privileged business license will be required to complete the Authorization to Release Information and provide two (2) original, notarized-signature forms giving authorization to the Las Vegas Metropolitan Police Department to conduct a background investigation.
Key Employee Designation Form
Each applicant for a privileged business license will need a letter from their employer designating the individual as a Key Employee. The letter should include the individual's job title, business name, location, business license number(s), business phone number and contact phone number. Alternatively, the Key Employee Designation Form may be submitted in lieu of the Key Employee Designation Letter.
Metro Application Memo/Instructions
The Metro Application Memo/Instructions contains information from the Las Vegas Metropolitan Police Department on completing their required forms (Personal History Form, Personal and Financial Questionnaires), fingerprinting requirements, how to apply for work cards, alcohol awareness training and health cards.
Personal History Form (PHF)
Each applicant for a privileged business license will be required to complete a Personal History Form detailing their personal history, including personal references, legal history, family connections and history, and past residence information.  This form requires notarized signatures in several places. A color, passport-sized photograph of the applicant is required to be attached to this form.
