PERSONAL PROPERTY FORMS

Aircraft SCRA Affidavit

Under the provisions of the Servicemember's Civil Relief Act of 2003, active duty servicemembers can file for an exemption on their personal aircraft.  The form can be printed from PDF format.

Personal Property Change Form(new and existing businesses)
The responsibility of keeping the Assessor informed of any changes in the business address, location or mailing change, is that of the taxpayer.  The new business information form is used to add a new business or to change information regarding an existing business.  The form can be printed from PDF format.

Installment Payment Request
Request to have personal property taxes billed in installments.  This form can be printed from PDF format.

Abatement Appeal
Nevada Revised Statutes provides taxpayers with the right to appeal through a written petition form for partial abatement of property. Call 702-455-3891 for more information.

Clark County Board of Equalization Agent Authorization Form
Nevada Revised Statutes provides that at the time a person who files an appeal on behalf of the owner of a property, the person shall provide to the county board of equalization written authorization from the owner of the property that authorizes the person to file the appeal concerning the assessment that was made. If the written authorization is not provided with the appeal it may be provided within 48 hours after the last day allowed for filing the appeal. Clark County Board of Equalization Appeal Form Instructions

Exemption - Surviving Spouse Affidavit

Mailing Address Change Form - The Assessor's Office maintains the official real property mailing address records for Clark County. The form can be filled out and submitted in PDF format. 

Electronic Payment Application
The Assessor's Office offers the option to pay personal property taxes with an automatic withdrawal from your checking or savings account. Explanation of how the program works is on the Manufactured Home page. The form can be printed from the PDF.

Homestead Declaration
When you record a Declaration of Homestead, Nevada law protects the equity in your home up to $605,000. A brief explanation of what the law protects and does not protect is listed on the Homestead page. The form can be printed from the PDF.

Low Income Housing Application
Nevada Revised Statutes provides for exemptions to developments that provide low income housing to residents and meeting certain requirements. The form can be printed from PDF format.

Manufactured Home Conversion Affidavit
Manufactured home owners who own both the home and the land on which it is located may be eligible to convert their manufactured home into real property. A conversion requires the issuance of final inspection of a building permit. The affidavit can be printed from the PDF.

Owner's Release of Confidential Information
Authorizes the Clark County Assessor's Office to release personal information that has been kept confidential pursuant to NRS 250. The form can be printed from the PDF.

Pollution Control Affidavit
The Nevada Legislature provides for property tax exemptions on property that is used as a facility, device or method for the control of air or water pollution. For more information on the requirements of this exemption, please call (702) 455-4997. The affidavit can be printed from PDF format.

Property Exemption Application
The Nevada Legislature provides for property tax exemptions on property owned by religious, educational or non-profit organizations meeting certain requirements. For more information on the requirements of this exemption, please call (702) 455-3882. The form can be printed from PDF format.

Penalty Waiver Request
Request to have the penalties waived for a delinquent personal property account. Attach any backup documentation and mail with the completed form to the Assessor's Office. The form can be printed from PDF format.
