• 1 Instruction Sheet
• 1 Original Writ of Execution (with raised seal) and 2 copies
• 1 Original Writ of Garnishment and 1 copy
• $5 check or money order made payable to the place of service (usually the bank or employer)
• 1 Notice of Execution
