Effective Monday, June 6, 2022, the Division of Air Quality will resume normal operations and no longer accept compliance documents/reports electronically or via email. Instead, these documents must be delivered directly to DES’s offices at 4701 W. Russell Rd., Suite #200, in person or via US mail or courier (i.e., FedEx, UPS, etc.). Each form must be accompanied by a Truth and Accuracy certification statement. A Universal certification statement form is available below under Compliance Forms. 

In browsers other than Internet Explorer download the form by right-clicking the link, selecting "Save link as" or "Save target as" and opening it with Adobe Acrobat Reader.
