Source Testing Forms


Effective Monday, June 6, 2022, the Division of Air Quality will resume normal operations and no longer accept testing protocols, gasoline testing notification, testing reports and gasoline testing results electronically or via email. Instead, these documents must be delivered directly to DES’s offices at 4701 W. Russell Rd., Suite #200, in person or via US mail or courier (i.e., FedEx, UPS, etc.). Each form must be accompanied by a Truth and Accuracy certification statement. A Universal certification statement form is available here.
