Incident Records Request Forms and Fees
Fire Incident Reports are a written record documenting that CCFD personnel have responded to a call.  There are two types of incident reports:  Fire and Medical. Fire reports are public record, however, medical reports can only be released to the patient or individuals the patient designates with proper documentation. Incident reports and Medical Releases will be processed within 30 days of request.

Incident Reports 
 $0.10 per page
COR Certifications  $5.00 
CDs or Photos   $15.00 
Witness Fee   $26.00 


The Custodian of Record will contact you regarding the amount owed  after your request is processed. Please do not send money until we send you an invoice for payment.  We are unable to process refunds.

Forms of Payment:
Exact cash amount or check made payable to “Clark County Treasurer.” Payments can be mailed to 575 E. Flamingo Road or dropped off between the hours of 8 a.m. and 4 p.m. Monday through Thursday. Once payment is received, your report will be faxed to your attention or obtained by you from our office.

Document Destruction Policy
As per NRS Chapter 450 B, reports and other records documenting fires, emergency medical incidents, and fire related investigations will be retained for six (6) years from the date of the incident.
