Alternative Health Care

The Alternative Health Care (AHC) Program facilitates the transition from hospital to home, supporting clients in returning to independent living. AHC is a 3 month program for individuals that have discharged from a hospital, nursing home, or rehabilitation center within the last 30 days. If the individual needs care following the 3 months, and they meet the HHHA financial criteria, they can transition to the ongoing HHHA program.

Services

  • Personal care services (bathing, dressing, grooming)
  • Grocery shopping
  • Prescription pick up
  • Laundry (including linen changes)
  • Light meal preparation
  • Light house cleaning (limited to ½ hr per week)

 

Eligibility

  • Over the age of 18.
  • Not eligible for similar services through insurance or another program.
  • A resident of Clark County.
  • Disabled and/or have a medical need for services.
  • Submission of income and asset documentation to meet eligibility requirements.

 

How to Apply

Referrals to the AHC program commonly originate from care settings such as hospitals, rehabilitation centers, or skilled nursing facilities. Nevertheless, any individual is eligible to make referrals.
