The CARE Homeless Outreach Team (HOT) works in collaboration with various community partners including the Las Vegas Metropolitan Police area commands to provide outreach services to the unhoused population throughout Clark County. This program is committed to conducting comprehensive assessments, facilitating service connections, referrals and offering essential resources and assistance to individuals in need.

Through our outreach efforts, we aim to stabilize those experiencing homelessness by addressing immediate needs such as emergency shelter housing, health, transportation, and heat relief through the Clark County Cooling Stations. By engaging directly with individuals in the community, we strive to build trust and establish meaningful connections that can lead to long-term solutions.

The CARE Bridge to Home (B2H) Program serves Clark County residents on the path to housing stability. The CARE B2H Program provides short-term housing and case management to individuals, couples, and families transitioning from shelter settings into permanent housing. This 90-day transitional model focuses on self-sufficiency, goal setting, and long-term housing stability. Bridge to Home follows an empowerment-based approach. Case managers help clients identify and reach their own goals by building confidence, stability and independence.

The B2H Teams may assist in responding to CARE Inbox referrals. The teams are also available to support individuals and families displaced by mass closure or property damage, offering essential resources and referrals to support recovery. B2H teams participate in community events to share important information and connect people to available resources.
