Requirements

• Submit a W9
• Provide copies of all current business licenses and certifications.
• Provide copies of emergency evacuation procedures.
• Pass initial and regular facility site inspections conducted by CCSS staff.
• Maintain personnel files for all facility staff, including status information to document employability according to the DHS.
• Overall cleanliness appearance of the facility/residence.
• Complete purchasing required documents (if applicable)
